Please print and bring with you the Tryout Evaluation Form (Find this under the forms tab). Fill out legibly and completely the top portion only.
New to BV, bring athlete's birth certificate
First time attending one of our events? Bring printed and signed liability waiver form (find under forms tab)
After evaluation is complete at tryouts, we will notify you at tryouts of a Team Placement. A non-refundable $500 commitment payment will be due at tryouts to confirm the placement. Be prepared to make this payment at tryouts. Check, cash, credit card. If payment is not received, the player may not have a position on the team. All payments are non-refundable.
