By paying the commitment fee you acknowledge that you have carefully read the Contract and you agree to all aspects of the contract and liability waiver.
We need:
1. a digital copy of the signed signature pages of the contract (there are 2 pages)
2. we also need a scanned image of your athlete's current insurance card.
3. If you are new to our BV family, we need a copy of athlete's birth certificate to verify age
