By paying the commitment fee you acknowledge that you have carefully read the Contract and you agree to all aspects of the contract and liability waiver.
We need 1. a copy of the signed signature pages of the contract (there are 2 pages) and 2. we also need a scanned image of your athlete's current insurance card. 3. If you haven't played for us previously, we also need verification of your athlete's birthdate. Birth Certificate would be fine.
