Please Go To the "Forms" tab under "More" and click on the 2018-2019 Season Contract. Please read it carefully, print out and sign where appropriate (2 pages), scan and email back to director@bulverdevolleyball.com. There are 2 signature pages. One for the contract and one for the liability.
By paying the commitment fee you acknowledge that you have read the Contract and you agree to all aspects of the contract and liability waiver.
I also need a scanned image of your athlete's current insurance card.

Future Payments will be due as follows: September 15th, 2018 October 15th, 2018 November 15th, 2018 December 15th, 2018 The amount due will depend on which level team you will be playing on. Details will be emailed to the teams.
